I only have an e-stim cock cage with urethra insert . I do not use it often . On the lower to moderate settings it is a pleasurable pain . The very top setting moves into real pain and I limit the time there . My experience is that the basic “TENS” units give the highest amount of different options to find the right vibe for you. They aren’t expensive either. With this you can buy penis rings, anal insertable, and the plain sticky white pads are also very fun. It’s really fun figuring out what works best.
