If you get the right cage there should be no discomfort so long as you don’t have an erection.
I wore my cage during all my waking hours for over 4 years and during Locktober I wore it 24/7 for the whole of the month.
The point of being in chastity though is tease and denial. The keyholder is meant to stimulate the person wearing the cage but not allow them to come. The longest my wife denied me was 6 weeks but she would provoke an erection every day and that’s where the discomfort comes in.
Being erect in the cage, if it’s the correct size, is constricting and uncomfortable but should not be painful. The “problem” arises though because the constructing feeling is stimulating in itself so once she got me hard it became self generating and I would stay hard in the cage until I could think myself soft again. You get fully hard inside the cage but not to your normal size as the cage restricts the size to which you can grow.

I know you’ve done this before but if I might give you some advice.
The idea of your wife’s friend carrying the key to your cage is really appealing but make sure that you have access to an emergency way of getting out should you need to.
You’re going to be locking something which regularly changes size and shape inside a solid rigid container.
Sometimes things get trapped, snagged, chafed or nipped and you need to be able to get out in a hurry.
