I'll second Adam's recommendation. We've tried both the Doc Johnson Ultra Harness 3000 http://www.lovehoney.co.uk/product.cfm?p=4751 which is the one with the opening below the dildo so it can be worn by either partner, and the Fetish Fantasy one that Adam describes. Both worked fine, but I preferred the FF one as you could tighten it more securely. It's also useable with a wider range of dildos and is considerably cheaper then the Ultra Harness.
There are also cock ring-type toys with added anal attachments, which might be worth a try, such as this one: http://www.lovehoney.co.uk/product.cfm?p=14024. I haven't tried that one though. I can't find the one I have at the moment, but it's a jelly-style ring with a vibe bullet on top and a semi-rigid dildo that sits underneath. It's not that thick, but Herself really likes it and you can adjust the shape and angle of it.
