With regards to point 3 you could always use some post orgasm torture/torment. Basically, after he has a full orgasm just keep going and don't stop either with your hands, stroker, mouth or a combination. After orgasm, guys are supersensitive and you may find he will be overwhelmed by pleasure that feels too good and he begs for you to stop. Might need to use some cuffs or restraints though.
