At the end of the day it’s all going to come down to personal preference, but I would probably go with the regular - just to ease the step up in size. If it turns out that’s super enjoyable, you can always add the girthy to your toybox later. I know in our case Mrs. has no interest in adding any more girth, and we use the extender EXCLUSIVELY in conjunction with her favorite vibrators to a) reduce sensation for me, and b) allow different positions to create room for the toys without slipping out.
