Our experience is with using TENS/EMS units with various electrode configurations for pelvic floor exercising so not really aimed at being erotic in its own right though we can appreciate how it very well could be.
The first thing to appreciate is that it does not have to be painful at all. Large area electrodes tend to reduce the local skin sensation and increase the degree of muscle contraction the stimulation causes. There are also fairly complicated effects of pulse length, current levels and pulse repetition rates that affect the sensation and muscle reactions and can be endlessly experimented with even on basic TENS/EMS units. For my PF exercises I now use a metal butt-plug as one electrode and a metal kick-boxing shield as the other and with these large electrodes there is almost no local sensation at the electrodes but considerable muscle contraction that feels rather like having the butt-plug slid in and out by an unseen hand - a far from unpleasant sensation!
